Glass in Special K cereal?

Did anyone see that Special K cereal is recalling boxes because of some glass? http://www.cbsnews.com/8301-204_162-57570278/kelloggs-special-k-red-berries-recalled-for-glass-fragments/ How does something like that happen? Surely there are standards that prohibit glass of any kind in the production area so how does something like that happen?

Is it just me, or does anyone else stay away from all products of the same type when one is recalled? I have a box of Red Berries on my desk that I just checked the UPC codes on. Mine wasn't on the list, and I'm not even in the U.S., but this box is going in the trash regardless, and I probably won't buy any cereal for awhile.
