Earlier this month two competitors in the Nautica New York Triathlon died.

I can say that the swim is the most anxiety producing of all of the events in a triathlon for me.

I think they should have to sign a waiver to enter the race even if they don't disclose their medical history (there would likely be a privacy issue up here in Canada about that).

I think having emergency flags given to participants that they could raise (even a red handkerchief) in case of distress and adequate number of rescue personnel on the swim course might be helpful.

In the women's only tri's that I have done they have "swim angels" women who will swim with anyone who requests it.
