I don't have anything to add that the others haven't already mentioned.

If he disrespects you in public, it's hardly going to make you feel close to him. So that needs to be addressed. At this point I think that with him threatening separation, then you need to suggest counselling.

Well, I have something to add. I think the lady should get to work on getting herself in better shape in order to improve her health.
Once this is accomplished, she will find that there are many things she wasn't interested in doing, that will be far more viable after losing weight.
Lastly, once she is in shape, she can work on sheding the other 150 to 200 pounds she has been saddled with in the form of her "dear" husband.
