Maybe rather than putting so much focus on yourself now, you should try to explore why it is that he started smoking again in the first place. As a smoker I know the sure way to NOT get me to try and quit is to nag me about the effects of smoking. I know those and I'm sure your H does too. A former smoker who picks up the habit again generally is going through some type of stress that he/she can't deal with so goes back to what they think works ie. smoking.
Get him to talk to you about the underlying issue. Do this without telling him he stinks, he has bad breath, or that the kids don't want to be near him. He knows this stuff. Be supportive and loving when you talk to him about it.
