The only thing I am thinking is; is she using her maiden name to get state aid? There are many articles on how people beat the system and they seem to know how to do it..

Being that she'll do anything to screw you over as I remember........maybe she just went out of network to annoy you so you'll have to pay more than for a doctor IN network.

If you think she signed up for Medicaid, even though you're paying for insurance, of COURSE you should report it.
