Read Relationship Resuce by Dr Phil. There are exercises in the book to determine what is important to you in the relationship - one is about physical appearance, maybe you can get your guy to look at that part and be honest about your weight gain.
Since it's an issue for you - the weight - what are you doing to change it? Working out, running, change in diet - if nothing is working maybe it's time for a physical to make sure it's not hormonal or thyroid-related.
As for your self-esteem, consider some self-help books (see list below) and/or some short term counseling. I also recommend you have a heart-to-heart conversation with him about the changes and explain that because of these changes is the reason you have a hard time with is visits out of town.
