male in peace needing advice

So here's the story.
I m acquainted with this girl, very pretty, gifted and good natured. One night about 18 months ago my friends and i were in a crouded bar and she was there too. She came over to me and kissed me enthusiastically on the cheek upon arriving (too my suprise as we don't know each other that well). She then picked me out of the croud with her eyes and smiled at me several times over the course of a couple of hours. I at the time, and to my great regret now did nothing, because i was in a relationship with another girl. This relationship eneded soon after but not because of the girl in question. When I left the bar that night I went to say goodbye to her and her response was an audible sigh of disappointment. The next night I saw her out she gave me what i consider to be a very dejected/disappoited look.
Over the course of the nexst 18 months i ran into her several times and she was very shy and the situation between us was incredibly awkward. At parties, she would go so far as to walk away from me as soon as I entered her/her friends personal space. Whenever we meet she avoids eye contact at all costs but does look at me when i m not looking. It is impossible to get her and keep her talking. I presume this is because of my past inaction on the night i described above. Would the ladies on the board behave in a similar way around a guy they liked but thought was disinterested in them/ behaved as i did?
In light of the fact that its so awkward between us now and an inability to engage with her in casual conversation i feel the only course of action is to ask to talk to her one to one. SHould I Tell her my regret of not responding to her signals and that i do like her and then see how it goes from there? Do you think this ship has sailed or is it worth going after?
I guess i need female opinions more than anything else but all help would be most welcome.

It seems she really liked you and went out on a limb to let you know she was interested.

a man and be honest with her.
