For me, "healthy FWB" is an oxymoron--because I can't remain emotionally detached from someone I'm having sex with, so any FWB or casual arrangement with an ex or otherwise ends up not being healthy for me.
So I think the first question to ask yourself is (and you need to answer honestly), can you really maintain the emotional distance you need to in order not to get hurt if you do this? If you can't, no ground rules are going to keep you safe.
i'd just cut it out.. it's just bad news! if you're like me, you'll learn this lesson after doing it once and never look back... that one time made me feel so horrible afterwards that i'd never do it again. learned that lesson the hard way!
