When a guy says he's infatuated with a woman, exactly what does he mean??? Is he in love? Is he attracted to her sexually? Is he fantasizing? What is infatuation to a guy?

Honestly, I don't think I've ever used that word. But I agree with the others, it would something superficial, like a crush.
It means he is strongly attracted to her on a superficial level. He's seen her around and perhaps associated with her a moderate amount. She has the right look and hasn't said or done anything that turns him off--at least as of yet.
More time in each other's presence, deeper conversations, etc. are needed to see if it is love.
Can't speak for all guys when I say this, but to me it means a crush. That's one of my main weaknesses, I get a crush on a woman for sometimes next to nothing. To me it is a desire for anything her. Conversation, sex, lunch, whatever. The infatuation usually fades quickly depending on the circumstance, how often I see her, type of personality, soon as she starts talking..
seriously, I'm sure each man has his own level, but for me it means crush.
