Your process will differ based on your surgeon and your insurance.
Each process differs by both insurance and the bariatric center because they each have their own requirements.
My process consisted of being referred in December 2007 by my PCM. The bariatric center called me in January and scheduled me for an informational seminar which you are required to attend before you see the surgeon. My seminar was scheduled for March (12th, I think). I saw the surgeon around the 21st of March 2008.
My insurance didn't have any requirements other than being 100 pounds or more overweight which I was. The bariatric groups requirements consisted of:
1) Attending 3 support groups and having a card signed by the leader as proof that you attended
2) Meet with their nutritionist about the expectations/plan
3) Lose 5% of your body weight (from your first appointment)
4) Have a Psych evaluation done
5) Bloodwork/EKG (Depending on history)/Sleep Study (Depending on history)
Once all of the above requirements were done then they submit your authorization to your insurance company. My auth was submitted around June 5th, 2008 and I was scheduled for July 28th. I could not have surgery that day however b/c my co-worker was on vaca that week :( so I had to wait an extra month until August 27th.

FYI- I find that the people who are AGAINST WLS (Doctors/family/friends) are truly not educated about it. If they attended one support group meeting or seminar I'm positive their opinions would change.

Glad you have started the process! I can't imagine a 5 hr apt but at least everything would be out of the way then!
I was told to take 2 weeks off work and I did. I only work part time though.

It definitely depends on your job. If it is physical you may need more time off. I work in an office and I took 2 weeks off which worked well for me. I was super tired and needed to nap those first two weeks after I went back to work too.
