When you said this guy and yourself are "only friends," you mean that this is someone you talk to, hang out with, things like that? I wasn't getting the impression that he was a friend with a benefits, a guy who is your friend but someone you have sex with him. Do you two discuss sex together? To me, it sounds like this is someone who is considering having a sexual relationship with you; otherwise, why is he telling you that the doctor says he can have sex again? It's the "all things considered" part that is getting me. It's almost like he's thinking that since he's well enough for sex and you're his friend, so the two of you may as well have sex. At least, that's how I'm reading it.

Is this someone you're attracted to? Have you ever possibly given him any indication that you would want to have sex with him? Has he ever let you know that he's attracted to you and might like to have sex with you?

Well, if he broke the shoulder on the side of his dominant hand (is he right handed or left handed) and now he can have sex again, maybe it means that he can listen to the hand of one hand clapping again ;)

Maybe you should wish him well and refer him to the solo sex board.

I forgot to say in my original post that I thought he broke his shoulder during the act of having sex, like maybe he fell off the bed.

I highly doubt he would admit he masturbates to

I don't understand what part of the message makes you think he broke his arm while having sex?

Do you feel that you are close enough friends that you can ask him what he meant in the e-mail? Or maybe send him a reply saying something like "Congratulations. Do you have a partner in mind?"
