I am 20 years old and in a monogamous relationship and have been for over 3 years. He is the only sexual partner (intercourse) I have ever had. We are both adament about being careful. I am on the pill, he wears a condom, AND he pulls out. Is that ridiculous? Sometimes I think that, if we have to do that much to rationalize having sex, maybe we shouldn't be doing it. I go back and forth on the issue, but I just always freak myself out about accidentally getting pregnant..even though I know we are safer than many people I know.

NO, more is not better. You're not able to actually enjoy sex with all of this worry :)
Being on the pill (if you are taking it correctly) has you fully protected - all of the other is unneccesary at this point.
Unless you are concerned about STDs - then that's where the condom comes in handy.
The chances of you falling pregnant when using the pill and condoms (and no withdrawal) is a ridiculously low 0.0006%! So while I can understand your concern about not getting pregnant, and wanting a second method for back up, just in case, I think the withdrawal is perhaps a little bit overboard ;-).
Stopping to put a condom on is a bit of a mood killer in itself, but to have him pull out at the end as well, that must take some of the enjoyment out, to be sure lol. Sex is to be enjoyed - if you're going to all those lengths to avoid a possible pregnancy (which has next to no chance of happening), then I can't see how you can get complete enjoyment out of it ;-).
