I voted for Susannah.
I think it depends on how you are saying the name too.
Susanna with a long a I would spell the first or second way.
Susana with a short a (sus-ahna) I would spell with only one n.

I have to somewhat disagree with some of the previous opinions. If you're going for an old-fashioned feel, Susanna is actually the most common spelling. The added -h ending is a variant of that.
Susanna -- this spelling -- has been in the top 1000 every single year since 1880. Susannah, on the other hand, has made the top 1000 less than 15 times in the last 125 years. It didn't make the list at all between 1888 and 1966, and hasn't been in the top 1000 since 1978. Susanna is clearly the most common, familiar, classic spelling.
Susana changes the pronunciation, giving the second "a" an "ah" sound and inviting people to say it soo-SAH-na, rather than Susanna. I would only use this spelling if you prefer that pronunciation and/or want to honor a Spanish or Italian heritage.
Like previous posters, I don't like the Z spellings at all. They're much too phonetic, making it seem like you either really wanted to make sure people would pronounce it correctly or simply guessed at the spelling. I'd avoid them.

I like this name a lot - it's very pretty but not at all cutesy. It will also work well from infancy through adulthood. Great choice! Will you call her by her full name, or use a nickname (Sue, Susie, Anna, Zanna, Sadie, other)?
I voted for the Susannah spelling, but to be honest, I like both Susannah and Susanna. I don't like the Z variations nearly as much - for some reason, they look a bit harsh. I agree with the other posters about Susana - if you want the name to be pronounced Su-SAH-na, this is the way to go; otherwise, I would go with Susannah or Susanna.

I like Susanna & Susannah equally. This is one of the few names where two spellings look right, imo.
As for the name itself, it's lovely. It's old fashioned but refreshing. It works well on any age, & it has some cute nn options. Susie, Sassy, Sally, Sunny, Anna, Annie, etc.

If you want the "ann" sound I'd spell it Susanna.
