Well I had a lovely night last night...NOT. I ate a cube of hard cheese and an apple before bed. I spent the rest of the night peeing. SIGH. Should I have taken ANOTHER metformin? I already had two of them and BS was all over the charts all day on limited carbs. This is getting really hard to deal with. As long as I avoid carbs I am fine, but then again I risk malnutrion. Any ideas on a safe bedtime snack? Anyone know if ketones are processed like glucose? This is the only explanation I can think of. Ketones function like glucose in the brain but does it actually have an effect on your BS?

The cheese should have been okay - I'd've left off the apple and maybe had a couple low carb crackers - not more than 10 or 15 carbs max (an average apple is around 20). I've done okay with just cheese (string cheese is pre-portioned which is helpful) or with a low fat yogurt (plain yogurt with a spoonful of jam made with Splenda for instance).
As you note, carbs are everywhere and everything has a little bit - the trick, such as it is, is to stretch them out AND get plenty of exercise (20-30 minutes per day most every day). So, instead of your bedtime snack being something 'extra', you'd want to divide out your dinner and maybe keep a serving of carrots off to the side to eat for the snack. Not that you'd necessarily cook them and then let them sit but the idea is to look at dinner and say "Okay, instead of 45 carbs at dinner, I'll have 30 and have 15 later before bed".
For me, unless I'm feeling/testing low, I don't do a bedtime snack at all. I do take my metformin in the evening but I just make sure not to lie down for a bit afterward and it's not a problem (YMMV) so having it with food isn't 100% essential. For that matter, if I do take it right after dinner, I'm more likely to have a bit of stomach upset than if I take it halfway between eating and sleeping.
What might help is keeping a food log (if you aren't already) and note the items, the calories (if weight is an issue), the carbs, and anything else related (when you took meds, any exercise prior to bed, etc). Then you'll be able to see "hey if I have 2 ounces of cheddar and 2 low carb crackers, I'm great but if I have an apple with the cheddar, not so good. And if I have a graham cracker with peanut butter it's okay most of the time." and so on.

There isn't a diabetic out there that at sometime in their disease doesn't get afraid to eat carbs because of the effects on their blood sugar. What really is happening is that there is nothing wrong with carbs but rather that the diabetes has gotten worse and isn't being adequately treated. This is such a slow process we can't see it happening until it hits us over the head.
I have had this happen to me several times in the course of 17 years with diabetes. So what do I suggest. I suggest that it is time to talk to your doctor again about your treatment regime. See your doctor armed with a glucose diary and a food diary.

You just told me what I already know but don't want to know. An apple should not have that effect unless my insulin production is decreasing. DAMN.

There are six classes of drugs to treat diabetes now so you aren't limited to sulfonylureas and metformin to treat diabetes. So I would suggest that you talk to your doctor about some of the new meds, specifically byetta or januvia. If you don't want to take injections Januvia would be choice. Of course there is also Actos or Avandia which have been around longer but I personally don't like these drugs and refused to take them. The other two classes of drugs are insulin and Symlin. While I am on Symlin, which you have to take by injection before each meal I am not sure how widely this drug is used. And isnulin I am sure you already know about and from what you have said I don't think it would be needed for you right now, IMHO! Just remember I am not a doctor and so I can't make a recommendation.

YIKES. I had no idea. Now that you mention salad, a woman who lived next door to me died after eating at a salad bar in California. I break out in hives on sulfa and have not touched in since I was 16.....a LONG time ago.
