Metforim, Januvia, something else?

I think I am about to the point of needing to do more than diet. I want an idea when I go in what route I would like to go.
Doesn't metforim cause lows? Januvia doesn't though does it? Can you take it be itself? I suppose byetta would be a possibility but I don't know if I could do shots. I know some of you do now...........
My numbers scare me. I can't get them back down. The last few days they have been higher than I like. REALLY higher than I like! This morning, It was 130. Before lunch it was 140. Think before supper it was 159 and about 3 hours later 189. And that is without eating any carbs inbetween! And eating pretty darn healthy! Breakfast... 1 scrambled egg on a ww muffin and a slice of cheese. I did later have an atkins drink with 2 net carbs. For lunch I had a crab cake, no filler, a lettuce salad with blue cheese dressing, left over steamed cauliflower and a sugar free jello. Only thing in between was swiss cheese. For supper, a prepared chicken kiev, I can't remember but it think 11 grams of carbs. I bit over a serving of brussel sprouts (9 grams) and a slice of tomato and about a 1/2 cup of cottage cheese (5 grams of carbs. And sliced peaches for 5 grams too.
I have noticed before that if I don't eat enough carbs my glucose is up. Could I have done that?
I hurt my leg about a month ago and have not been able to be on it.. broke a bloodvessel and not pretty!
Thoughts, ideas?

Is there someplace (or one of y'all) where I can get a really short, couple sentences maybe, idea of the different meds (Januvia etc - I'm already on metformin) that might get suggested at some point? I slipped up big time the last few months (I went a whole week in September without my nightly walk - not good). My A1C done earlier this week was 6.4 - not awful but it's crept up from mid to upper 5 to 6.2 then back down to 6.0 and now to 6.4 - I know I can get it back to around 6 again (I've gotten back on track over the last few weeks) but I want to gather info BEFORE the doctor says he wants to put me on something or other.
One thing that makes me nervous about Januvia (which sounds pretty good in general terms) is that you can be susceptible to upper respiratory infections - I am already susceptible (had a nasty upper respiratory infection when I was 12, now every head cold that comes by lodges in my chest and there was that nasty bout with pneumonia right before I was dx'd in Dec 2006/Jan 2007). So, what kinds of info can y'all give me in simple terms?

This was just one of the incidental findings and doesn't occur that often but they needed to report it. I haven't heard of anyone not being able to take it because of previous respiratory infections. I didn't notice any for the 15 months that I took it.
My new endo has stopped the Januvia because she thinks my blood sugar is under too tight of control. I am not agreeing with her! I ahve noticed that my appetite has returned. DRATS!!

I am no doctor but please don't start the metformin at 500 twice per day. You will be one sick cookie. Please ask your doctor to let you start at 250 once per day and work your way up to the 500 twice per day.
You A1C is wonderful. You are more than meeting the treatment goals of 7 or less. You are to be congratulated.

Susan, that is why Mary Frances suggested you call your dr and start at 250 mg once per day!
