Hello everybody,


                                    My name is Amanda and I'm 24yrs old DH is 23yrs and we're trying to conceive #1. Our ttc journey started soon after being married in April last yr, after 10mths of BFN we decided it was time for him to do a S.A. The lab results were not specific, there was no number telling us his total SC only that there were few motile sperm and apparently it was so low they couldn't test the morphology etc. I was pretty shocked but just glad to find a possible reason for us not being able to conceive yet.


                With no numbers attatched on the S.A. results I assumed that low meant at least 5-10 million. Our medical doctor started him on Proviron and vitamin E supplements. Our M.D. told us that we would have to wait 3mths to see if it was working. I got impatient and started to seach for an R.E. here {in Trinidad a very small Caribbean island} I found out that there is only one fertility clinic and it was opened in 2004. {not that long!!!}


                     I had my first R.E. visit on the 30th of June 2010 and it was pretty horrible my R.E. made alot of assumptions based on the first S.A. She sent DH to deposit a sample and told me when he left that they might not find any viable sperm and basically told me our only option might be using donor sperm or if he has a few healthy sperm we could use it for ivf. I was pretty devastated because I had my hopes set on doing a few cycles of iui but on our way hm they called and told us his count was 400,000 in total and indeed  ivf is our only option. My R.E. also suspected that DH has a genetic prob. and the low sperm count or no count at all could be passed on to our son if we have one. Plus here in this 3rd world country ivf is $60,000 more than a yr of my pay, to you all it may seem alot and to me it is, but this is equal to only $10,000 US.


            DH also did some blood work and they told us they would have the results in 4-7 days but 2 days after they called and said our R.E would like to see us. So I'm kinda worried but hoping for th best. I hope to get to know you all better...

Welcome. First, don't jump to conclusions. Wait to see what the RE says. I guess 400,000 isn't great but you need to know motility and morphography as well. As with most of the bloodwork and testing one single test does not give a full picture.

To put it in perspective - I'm 46 and for most women my age they would have not even tried with my own eggs and would have insisted on donor eggs but my RE was very progressive and he gave it 2 tries with my own eggs and the second time it worked and I am now 34 weeks pregnant - so anything can happen.

You need to discuss with your DH what you are willing to do to have a baby - is IUI doable? IVF? Donor sperm/egg, embryo adoption? There are many options available to you but you have to do what is right for the both of you

Hi,
Welcome to the board. I know things don't sound good right now but they can do a lot with ICSI. As long as they can find a few good sperm u should be all right doing IVF with ICSI. You have a definite plus in that you are young and your eggs are young. I would die to have 24 yr old eggs.

I would have loved to have 400,000 sperm, or even 40 for that matter. We are also dealing with severe MFI- 0 sperm count. The day before my egg retrieval, DH had to have a procedure to check his testis for any sperm in there and none were found. Fortunately, we had donor back up to fertilize my eggs.

ICSI is amazing, and with that count, they should be able to find enough decent sperm to use. Also, with a count like that, there may be some hormone treatment they can do to improve things, depending on how his FSH and testosterone levels come back.

There are a few MFI specialists that will look at your labs and things and give you a second opinion over the phone or through email. That might be worth it, depending on what the RE says when you go back.
