I have a fear of pap smears. I have to take Ambien before I get them done. I feel like my ob/gyn does not want to treat me anymore because my fear is so overwhelming. My family physician has offered to do one. I have not had one in two years. Why is this so terrifying for me?

If you are experiencing pain, is it due to the insertion of the speculum, or the actual procedure? If it is due to the insertion of the speculum, if you could find a doctor that uses a table, then what you can try is lifting your bum off the table a little as the doctor inserts it. This prevents your vaginal muscles from clamping up against the insertion, since the muscles in your body are working to hold your body weight. It makes it easier, and shouldn't be as painful like you are used to experiencing. Of course, this will only work if you're conscious through the exam, and able to do this.
If you experience anxiety over the procedure, then it's no wonder you find it an unpleasant experience. It might help to ease your concerns if you can have a preliminary appointment with the doctor first, so you can get to know them without the fear that your pap is about to happen. Have a list of questions to ask them, and make sure they know how hard it is for you to have these exams, and ask them to talk you through every step of the way.
Another option might be to learn to meditate, or learn relaxation techniques, in order to try to detach yourself from the process. I don't know how well this will work, but it could be better than nothing.
Of course, if you are happy with taking the Ativan, by all means ignore me :-).

I have read all of the responses on this thread and I think the thing that is not getting through is the whole weight issue--A good caring doctor with the right equipment can help you through this--do not give up. I am there with ya sister :)

No, you're not alone in experiencing fear during gyn exams. I also have pretty severe fear/panic during exams, but mine is from extensive childhood abuse and rape.
I have my fiance` come in and stay with me during the exam. He comforts me and I can hang onto his hand if things get rough. A lot of the abuse happened late/middle of the night so being sleepy and all doesn't help for me.
