Perineal pain in 2nd pregnancy

This may be too medical of a question to ask here (and I plan to ask my doctor but that's another week and a half away), but here goes.
During the delivery of my first child two years ago, I had a fourth degree perineal tear that still has me doing kegels just to maintain some kind of normalcy. But this new pregnancy (I'm 24 weeks) is bringing with it perineal pain that is quite strong at times and I can also tell that the overall strength of the muscles in the pelvic floor have weekened again, much like they were relatively soon after my first delivery.
I suspect that this could be normal due to the added pressure of this subsequent pregnancy but was just curious if anyone else has ever gone through this and could provide input. Anyone?

I don't have any experience with this myself, but the pulling and stretching as your body changes definitely sound like the culprit. You could try using some vitamin E cream on the area to try and keep the skin supple so it can stretch more comfortably, but other than that, I'm not sure what else you can do!
