Crisis Plan


What I am like when I feel well:


I am enjoying myself and the tings I do. I am not having panic attacks. I like to be around other people. I am sleeping and eating.


When I have these symptoms I am having a crisis. Other people need to take responsibility for me


Racing thoughts, self-harm/suicidal thoughts, disorganized thinking, voices/visions, thoughts of going off my meds.


Name and phone numbers...obviously you don't need that, but I'll let you know it's my pastor, family dr, psychologist and psychiatrist.

Perphenazine - 4mg 4/day - psychosis
Amitriptyline - 25 mg 4/day - depression
Clonazepam - 1mg 2/day - anxiety/sleep
Lamotigine - 25mg 4/day - mood stabilizer
Lorazepam - PRN - anxiety
Ponstan - 250 mg PRN - menstrual cramps
Arius
Ventolin
Flonase
Glucosamine
Omega 3-6-9
