From the blog of Dr. Gail Saltz....

Last week the FDA reviewed the data and decided to issue a black box warning on serotonin reuptake inhibitors, the most commonly used antidepressants. The warning is an increase in thoughts of suicide when first starting the drug. Such a warning already exists for use in children, but now the warning has been extended to 18 to 25 year olds.

(from my experience it is the fact that suicidality is already present in the depressed person but the motivation and energy to do anything about it are lacking, once the antidepressants raise the level of depression just enough to increase that energy and motivation but before the suicidality goes away that's where the risk comes in. They don't tell you that in the warning!)
