Do you have to be there at the exact time as printed on the fast pass? Can it be later than the time on the fast pass. I'm thinking what if we have dinner reservations the same time as our fast pass?

We've never had any issues with fast passes. A few years ago we were in DisneyLand and were sitting on a bench resting when a CM came over and asked us if we were having fun. Which we said yes (of course) so he said he was happy to hear that and gave us a handfull of expired fast passes to almost all the main rides and more for each of us (just my husband and myself no kids back then). We asked if we would be able to use them even though they were expired (some were even a few hours past the fast pass time) and he said yeah that it wouldn't be a problem and we didn't. Everyone at the rides never said anything to us about them being expired. When we were at DisneyWorld last year we did get a few fast passes and unforunately some had expired before we headed back to the ride but we were still able to use them. No questions asked. Maybe it depends on the time of year you go. Both visits were done in September/October, so maybe they didn't say anything because there weren't that many people? Either way, fast passes are great and I would recommend getting them if you can. :)

No, they shut the fast pass machines down (cover them up) when all have been given for the day. And you can only get a Fast Pass for the ride at that particular machine. Other machines will only give Fast Passes for those rides.
But you can always still line up for a ride the "old fashioned way" even when all Fast Passes are given out. The wait times for stand-by will be posted as always. Sometimes, all Fast Passes can be given out for a day hours before closing, but the wait time is only 15-45 minutes.
