1) Cardio

run

2) abs

bicycle and variations

3) chest

push ups and variations

I don't think there are many exercises I do the regular way anymore.

I've done some cool body weight flyes lately with clients and in class. You start out in push up position, hands on disks (or paper plates) and push them out and in. You add a tube for more resistance.

That sounds awesome.

That sounds really cool but I'd be scared to try it with my shoulder. :(

1) Cardio

Definitely elliptical.

2) abs

Lately, I've been doing pikes, roll-ups.

3) chest

Modified push-ups ('cause I'm not supposed to do real push-ups with the shoulder) and chest flyes on the ball.

Definitely. If they have shoulder problems (no one has anything as severe as yours), I have them do one arm and then the other. But, with your shoulders, I wouldn't do either option, either.

I like pike roll ups, too. I've had my class put their feet on the BOSU, hands on disks and pike up (lower level with knees on the BOSU). Or reverse, feet on the disks, hands on the BOSU upside down.

Interesting! I don't have discs and my gym is carpeted so I'm not sure it would work.

I'm glad you post your variations - I enjoy them. With working out at home and working with clients with what can fit in my car, I appreciate bastardizations of exercises. :)
