I personally don't say anything. There is so much lat pulldown abuse in every gym as it is. The only time I say something is when someone is sitting at the machine facing the wrong way.

It's such a dilemma because I work at the gym but I don't say anything unless I know the person and know they will want the feedback.

Funny, I didn't notice that but you're right. They kind of go hand in hand when you think of it. You can't pull behind the head AND lean back or vice versa easily.

If someone looks lost, I might ask if they need help. I don't know if I'd say something if they were using it completely wrong.

LOL It amazes me how often this happens despite the fact in our gym there are instructions-with pictures- on almost every machine.

Yeah, I see that all the time, too. I tell my class to reduce their weights (or go one arm and then the other) if they see themselves swinging like those guys outside trying to lift too much weight. When people concentrate more on how heavy they lift, rather than results, that's what they get. I see guys try to lift the stack and have never seen anyone do it with good form.
