Medical forms for daycare/camp/school

OK, if doctors are going to charge for filling out forms that take less than two minutes, involve mostly checkoff boxes and copying vaccination dates off a patient chart, they need to at least do them correctly!

Holy cow! That is terrible, especially the food allergy omission- that could have been a very bad situation :O you could probably complain to your dr, but would it help? In Canada we fill out our own medical forms and submit them with the registration so if there are any errors or omissions it is our own fault. I never thought about it before but I am now kind of glad it works that way- way more peace of mind.

Holy poop!!! I would be one fired up mama. wth?

Maybe you can write up your own "form" to include with the kiddos bc your doctors suck? or drop off a note to their teachers yourself?

Wow. I agree the dr should have included that info. I would sit down and make a list of his food allergies and send it in.
