I would think that you would need to disclose it, as you should any medical or psychological condition. I have a complicated medical history, and in addition to the form for the doctor saying that I am in good mental and physical health and am capable of parenting a child, I had to write up a medical history documenting any/all surgeries or hospitalizations in the past 5 years. I also had to write up any medications I am taking. I know that if you are in therapy you are required to have your therapist write up a short report as well. I understand your trepidation, but it really is completely reasonable for an agency or the state to expect you to share this information.

i dont really have much advice, i just wanted to mention that i also have DID and it is hard to deal with sometimes so i wouldnt leave it out of the paperwork. i am not saying that you shouldnt adopt more children i love being a mommy.
also if you have only recently discovered things then i would give yourself some time to process. i still cant be completely aware of what my alters will and wont do at any given time. i however am not interested in integrating because i think with young children it would be too much too handle right now.
if you are wanting to integrate i would talk mainly with your therapist and family about whether or not you would be ready to adopt during that process.
