transitioning to after 1 year

My LO is 11 months, and we're still exclusively breastfeeding (no formula), although she will eat table foods with us. No huge amounts though.

I'm planning to nurse until age 2 years. How do I transition to nursing beyond a year? Right now, she's nursing every 3 hours during the day, and about 4-5 times in the latter half of the night. During the day, she doesn't give any signs as to when she wants to nurse. I just look at the clock and offer, and she takes it. If I am sometimes delayed and wait too long, like 3.5 hours or 4 hours, then she'll cry, so that would be a "sign." After she turns 12 months, should I just wait until she gives me that sign, or should I continue to offer every 3 hours?

To be honest, I didn't really change anything after the 12 month point.

Jackson seemed to just change his nursing habits right around one year old.
