I use the Medela PISA and keep it on the lowest setting (there is only one setting).

I had a lot of issues in the beginning. My nipples were permanent cones for like the first 2 weeks after my dd was born. My areolas were just irritated red skin. I coundn't even tell were my areola ended and my normal skin began. Wearing anything was painful, but that was mostly due to the blistering from trying to breastfeed. Hopefully this isn't tmi but my nipples have never been huge, they are on the small side. So I was surprised when my LC gave me the big 36 mm horns! They actually turned out to be much more comfortable. Most of my areola gets suctioned in therefore it's not getting rubbed against the sides of the horns as much. Hopefully that makes sense. So for me, they worked much better than the 24 and 27mm. I also used olive oil during the healing process. It eliminated any friction. I don't think it matters which kind you use. If any makes it into your milk it would be so incredably minimal you don't have to worry about it. My LC at the time said it is not harmful to the baby. Hopefully you have already found something that works for you but just wanted to share my experience. Best of luck =).

I remember the beginning being very uncomfortable and even painful to pump. Especially in the first couple of minutes.

I had the same problem...I dreaded getting cold (if you know what I mean-sorry if thats gross) because I would be in such pain. My problem was solved by x-large breast shields as well as not using the Lansinoh; it literally ripped my nipples because it got too tacky. I found a product called Nipple Butter at Walmart and it's AMAZING! It's made with organic ingredients and you use way less than the Lansinoh. Hope this helps!
