Pain and sores- advice?

Hello-

I have suddenly developed incredibly painful cracks and sores on my areola (below the base my actual nipple) from pumping and could use any advice anyone has.

I use the extra large flanges for my Medela PIS, which I moved to while pumping for my older son. They have always worked great. I used to have friction and pain with the regular size flanges and these gave me the room I needed and all was well.

The sores started about 3-4 weeks ago. I thought it was a fluke and it would heal, but no luck. I am wondering if too much of my areola is being pulled into the flange and causing it to rub and then crack/bleed? Does this make sense? I can see a ton of it being pulled in, but don't know how much is the right amount.

I have been applying tons of lansinoh before pumping to grease the wheels, but it does not help. I know it is pumping and not nursing since it does not hurt to nurse and it tends to heal some on weekends when I only pump in the mornings.

I have been working and pumping for my 8 month old since I went back to work

hi! have you tried using olive oil instead of lanolin? i find that lanolin is too thick to properly lubricate the horns. Olive oil makes it glide more smoothly and less friction=less pain... but the lanolin works good if you apply it AFTER you pump. it helps heal the crack faster. i'm sorry this is happening to you... I EP so I pump 5x a day EVERYDAY, i would absolutely quit if the pain lasted as long as it has in your case :( i had a deep bleeding crack before but it went away after I re-positioned the horn slightly off-center so the cracked part wasn't touching the side at all.

also, check your suction, you may have it too strong. it definitely hurts when it's past the halfway mark in my case. you can try lowering the suction (or just until your crack heals) and just do compressions/massage to help you empty. good luck!

Thank you both for such great suggestions!

I will be trying the olive oil right away. I do notice the lansinoh just congeals when I pump- totally gross!

It is somewhat less painful at lower suction but I have been worried my output will be lower if I ramp down suction. Since I am at the 8 month mark, my supply has been dipping lately anway, making it harder to keep up. I am just barely pumping what he eats each day. Will I get the same output at a lower suction? Will it take tons longer to pump? I guess I can just try it today and see what happens!

I will also buy some smaller flanges and see how that goes.

I have had such an easy time pumping
