No, and it's something we really need to do. I just have a very hard time thinking that anyone would be willing to take on all four of our boys, especially with Devin's ADD, etc. It was an easy choice before when one of my brothers was still single, but now that he's married, it's makes it harder. I love his wife, but they already have 5 kids (one which is a relative of hers they are in the processing of adopting).
At least I know most of my siblings would be willing to take them on, even if it would be a burden. We are covered when it comes to life insurance, so the financial burden would be less. Thanks for posting the topic, it's really something that we need to get down on paper this year.
We don't either and it is something I worry about often. My parents are my natural choice but I really don't want to ask them to commit should anything happen. They need to retire and enjoy their golden years, not worry about raising children (though I know they would do it without question). My brothers are pretty much out too. My oldest brother is childless and intends to stay that way forever. My other older brother has no patience for my boys so that's not a good fit either. Larry's family is totally out too so I really don't know what we would do. Knowing this is a pretty big reminder to me that Larry and I need to take care of ourselves so that we can (God willing) be around for a very long while.

We asked my brother and sil years ago when the kids were small, but we never actually put it on paper, which is something we really should do.
We do have wills with guardians named, however, it's in dire need of an update.
