Disney and most major theme parks don't make those types of records
I don't know that it's necessarily the smartest thing, but I've told my kids if they get separated and can't find us at WDW to look for a CM (showed them, told them to look near rides, etc), but if they can't find one, to find a mommy with a little kid or a stroller and tell her they're lost.
From what I've read, if a child does get separated and appears lost, it's usually not even a minute before a CM is helping them. They know what to look for, and they're specially trained in what to do. The lost child stations are with the baby care centers.
When I was 6 years old I was separated from my 2 boy cousins and my sister at SeaWorld. I walked around crying for a few minutes and an employee came up to me and helped. She brought me to guest relations and paged my Mom and Dad. My Aunt and Uncle were also there and watched the other kids while my parents came to get me. It was scary for me being 6 but I was helped right away and I am sure CM's and other parents would help a lost child.
When I was at DW in September our party was getting off of the train and there was
We have been a bit paranoid about losing DD ever since DH lost her in the Disney Store (LOL!)at our local mall when she was about 3 yrs old
He was runing around the store looking for her - she was wandering around the mall looking for him. DH had just told the store personnel and they were about to lock down the store when a lady arrived from the other end of the mall bringing DD and the stuffed Disney toy with tag attached that she'd walked out of the store with!! This was how the nice lady figured out where DD was supposed to be of course.
Anyway, after that incident we have always told her if she loses sight of us she is to stay exactly where she is and yell "Mummy" or "Daddy" at the top of her voice until we come to her. While we were at WDW we also told her to look out for anyone with a CM badge and they would help her find us.
We haven't actually had to use this since then I don't think - but I do still remind her when we're somewhere really busy - even though she's now almost 9 yrs old.

The past couple of years, dh has given both kids one of his business cards to keep in their pockets (if they have them).
I lost my ds (then 6, now 11) in Magic Kingdom.
Instead of typing it out again, I thought I'd post the link:

Cast members are trained to help and are very comforting. However, my son turned himself in to a CM. He walked from Frontierland (by Splash) all the way to the castle by himself before turning himself in. I'm sure CMs will intervene if they see a child. However, I'm sure that busy times, especially during parades (like it was when Jake got lost) make it harder to spot solo kids.
My Tips:
1) Be careful during busy times and parades
2) Be careful when you have a large group -- don't get too comfy with swapping kids up.
3) Teach your kids to go to a CM - "the ones with badges"
4) Don't panic if your child is lost. Contact the first available CM.
5) Of course, make sure your child knows their name, your name, phone numbers

The other thing we did last time we went (DD was 7 then) was to take walkie-talkies. We carried one - she carried one(In a small back-pack or fanny pack with her water bottle). That way if she got lost she could call us and keep talking till we found her.
