I'm a notorious over-packer but our kids are close in age so I'll tell you what I bring.

I'd say my list is similar to the other posters. My kids are just now 6 and 4. Last time we went to the parks (for only 1 day), that was the first time without a stroller, so it was a bit different.

Assuming you have a stroller, bring an extra outfit in a ziploc bag - good for messes, wet rides, whatever may happen. Just put it in the stroller and you won't be bothered by it.

We have a fanny pack stuffed to the brim with 4 ponchos. We take it every trip. We've also always had a waterproof stroller cover and sometimes umbrellas. I don't know what we'll do without the stroller - probably bring the ponchos with us but maybe not into the parks if it looks dry that day. (Scary!) We just power on if it rains: ponchos for everyone, stroller cover, and some umbrellas, and we're good to go! :)

Otherwise, a few snacks, water bottle (used to do sippies, but probably won't again if we don't have the stroller), camera(s), bandaids. I try to pack as little as possible, since I'm generally stuck with the bag all day!
