When something is a priority to him, does he get it done? I think this is how you can sort out inability to stay on task from unwillingness to stay on task. If he struggles to get even the stuff he wants to do done, then I would seriously consider having him re-tested for ADD or some other issue. It's possible to miss a diagnosis.
Whatever you try, you're going to have to be consistent for a long period of time to see if it has any benefit. Sometimes it takes months to see any improvement when you're trying to change a behavior pattern and most people give up way before then.
Your son's symptoms are classic ADD/Executive Dysfunction.
I have to agree with re-testing with a current specialist. I know at different times the Docs are hesitant to dx Add/hd type problems during the episodes of media attention about over dxing kids and drugging them. Maybe your child was assessed
I agree with having him re-tested for ADD.

We had similar complaints about DS20 when he was in elementary & middle school. They complaimed about DD22 too but in her case, I got the "she likes to please" but she isn't that bright from some of the female teachers. It was very funny when the school saw her IQ test results.

The testing we had done was about 8 hours over several sessions by a behavioral psychologist.
