Since bio mom isnt in the picture, will whatever policies you set be enforced consistently? Or does she still enter the picture enough to make that challenging?

I suggest you pick one thing off the list and work on it. Try not to bring up any of the other stuff while you are working on that one thing. It's soooo easy to "and ANOTHER thing...." but I think they just shut down

So, pick an easy one to start with(probably not going to be the grade thing, huh)

I did a spreadsheet chart for my youngest, now 16. He was struggling not so much with chores and 14/15 but personal hygiene although we threw his chores on there as well. It worked remarkably well and, after the charts we made ran out, we didnt need to make anymore(8 weeks?)

There were not rewards or punishments-just the physical reminder worked for him but you may need to tie something in with your kiddo-they are all different!

You could have a jar with a supply of marbles and everytime he makes a put down comment to someone he puts a marble in the jar. Id do that for a week and then tie a consequence to the marble. Loss of ___________ for an hour or an evening, for example. It doesnt have to be huge-no need to stomp on the cell phone, however tempting.

I agree with the others - he's just 15, and even though his behavior is enough to make you crazy, it really is pretty normal at that age.
