People that wake up everyday to sunshine regardless of the actual weather, whose lives look peachy perfect b/c of their demeanor, whose energies and smiles motivate you or make you want to strangle them...

Or do you?

Bord: They could not offer my job to an adjunct because the adjunct could not deal with the graduate students. Fall: So there is NOBODY in this world that could take over your job? ------ Seriously Fall? She has already explained this 3-4 times. They would not give her job to a junior person, because of the grad student supervision. Think about it! You can't put a teaching assistant with or in the middle of a master's degree in charge of supervising PhD students. So, other people can do her job, as she readily said, but not an adjunct.

Oh ok, so your unique role can't be replaced. And being tenured guarantees you a job, whatever it is.

I would chose a resident or new MD over an N/P or P/A.  Sadly that's the direction some practices are going and that doesn't sit too well with me.

I cringe at "comparable" chestnuthooligan, You must already pay so much out of your pocket with your employer's plan then.

Jams, Bord in no way said that she couldn't be replaced. She said that she couldn't be replaced by a grad student or junior prof. She has explained this several times.

But the point is, if everyone had to quit once they had accumulated sufficient savings, that might happen. Highly-paid doctors ought to be able to accumulate our net worth all by themselves within a decade or so. Then it would be selfish to keep working, right?

A lot of doctors are ready to retire when the average person does, Why wouldn't they?  I'm sure a lot of that does have to do with being able to too but still..  There is a new generation of doctors ready to replace even their jobs when the time comes.....  I remember living in Indy and looking for a pediatrician when we started having babies, The doctor I found was the one my friend used and he was in his 40s (I remember the balloons and poster on his door when he celebrated his 50th, Lol!).  He is retired and can't be any older than 65 or 66 now.

"I would chose a resident or new MD over an N/P or P/A. Sadly that's the direction some practices are going and that doesn't sit too well with me." These are a lot of routine things I do not mind seeing an N/P or a P/A for. But I do want an experienced doctor around for backup.

"There is a new generation of doctors ready to replace even their jobs when the time comes..... " ------- Actually the US currently is short about 20,000 doctors and the shortage is projected to grow to around 90,000 by 2020. So they may have to find ways to encourage docs to practice past retirement age and find new incentives for people to enter the profession.

It comes down to comfort level, NPs are under the supervision of doctors anyway, apprentice roles if you will..... I see more of them in offices and hospitals but I think it is a cheap alternative to healthcare.. Maybe this will be all my kids know by the time they are my age, I really hope we can find a happy medium b/c when health is a matter of life or death I'd rather trust the advice of a doctor.

I don't need to see a physician to have a throat culture to see if I have strep or to have an ingrown toenail cut out, stuff like that, I have a physical about every three years and I do like having that "preventive maintenance" chat with the physician who's seen me regularly over the last twenty years.

You mean like to test for strep? The nurse performs that procedure on my kid and always has...

I compare doctors and P/A's to my sister's plumbing practice. She works for a team of specialized plumbing contractors. They are the guys that other contractors call when there is structural work that needs to be done because of some major problem. Every once in awhile she will get a call from somebody going through the yellow pages looking for a plumber to deal with a backed up toilet. She will tell them that they probably should just call toto-rooter or something, but they insist they want "the best." So they pay $300 for a $60 plumbing job. Same with a doctor doing something a P/A can easily do. I don't need a highly-trained mechanic to change my oil, a specialized contractor to unclog my toilet, or a physican to freeze off a wart.

So going forward and regarding healthcare, we should look at it like we look for a contractor or mechanic..  Hmmm...

Yes, Jamblessed, there are obviously certain parallels in any kind of preventive maintenance.

I don't think office jobs and being a people person are mutually exclusive.  I'm very much a people person ... And although I have an office job where the bulk of my work is on the computer, I interaction with anywhere from 10-20 people a day (in person) and another 5-20 via email or phone.

I just had an image of a doctor just out of med school doing a heart transplant or neurosurgery. ACK!
