I suck at grocery shopping. I lack any sort of organizational gene where grocery shopping is concerned. Add to that I hate it and basically the whole thing is a shambles. On average it takes me 3 trips to get everything I want (and that's usually because by trip 3 I just give up on anything I've forgotten at that point).
Just one example. Last fall, I was planning to make Chicken with Soba Noodles and Spicy Peanut sauce for dinner. I got everything I needed and on the way home from the Food Lion and realized I did NOT get everything I needed because I had recently broken my jar of minced garlic. Dammit. sigh. So I figured I'd make something else for dinner that night and make the chicken dish the next night after stopping at the Giant grocery store on my way home from work. And it wouldn't be an unplanned stop for once because I had to stop by to pick up my prescriptions so I'd have been there anyway.
Next morning, I go, drop off my scripts and do a little bit of shopping while I wait for the scripts to fill. I check out the little bit of groceries I bought and walk it out to the car. DAMMIT!! I freaking forgot to buy GARLIC!!!! sigh. So I go back in, get my scripts, BUY the damn garlic and leave.
That night, I'm getting out the ingredients for my dinner. And that jar of ginger? is NOT ginger. It's crushed garlic. I HAD garlic. All along! I HAD garlic. I did NOT have ginger!!!!! So I went BACK out to the Giant, got the stupid ginger, and made the stupid dinner.
I shop once a week at Stater Bros. About 1-2 times a month, I'll hit the 99cent store (paper products, cleaning products, foil/wax paper, etc.), Henry's (local health food grocery store. Should get there once a week but don't always.), Costco, and/or Trader Joe's. Unless I'm making a new recipe, I don't make a list as I buy the same basic stuff and decide what to make that afternoon for the evening meal. If we run out of something, dh will get it on his way home from work or I'll go when he gets home. There is a Ralph's just around the corner from us but it's a small one so I don't go there for the weekly shopping. Besides, dh's mom prefers Stater Bros. and we're her chauffeurs.
Costco about every 6 weeks, Trader Joe's about once a month (mostly for not-so-bad convenience food, a fairly small shop), and Safeway once a week for fresh f&v and milk.
I go about 2x a week and take a list. I only buy organics so if they don't have an organic version, I don't buy it (except for a few things like my jelly belly jelly beans. LOL).
I also go to farmers market when I can to get additional organics and locally grown stuff.
Once a week, usually on Saturday, I put in a Peapod order. (That's a grocery delivery service from Stop & Shop). At first I wasn't sure I wanted to "order" the meat and produce, but It really is worth it. I've got an out of the house career, a stay at home business, and 2 kids with a husband that also works, the LAST thing I want to do is go to the grocery store. (Besides, every time we go, the kids get another cold. I HATE those grocery "trucks" that every little kid has had their mouths all over!) I love Peapod. They take coupons, the delivery charge is well worth it for the convinience, the prices are still reasonable and I don't have any "impulse buys" at checkout. I even review my order in my shopping cart before I check out so that I know I "need" everything I'm buying. I order the groceries on line, get them delivered the next day. It's awsome. Roche Bros has the same service but it's more expensive.

I do a big grocery shopping every other friday/saturday (depends on our schedule).
