I think it was on Lake Superior, but didn't the Edmund Fitzgerald get swamped by waves? Yeah, I can't get the song out of my head, and all the Great Lakes are alike ;-).

Yes, that was Superior. It happened when I was in high school.

If by "man made" you mean "carved by glaciers thousands of years ago", then, yes.
