If you don't want to go with the IV, you can change the middle name or add a second middle name (maybe your maiden name?), and you can technically drop the Jr., II, III, IV and his name would just be his name. Hope that helps.

My DH is the 3rd also. We changed his middle name and Call our son by that. Instead of William Edwin he is William Cade.

My younger brother is named after our father without being a Jr. My father is Michael Ray, so they named my brother Michael Paul (small). Vaughn is another name that means little.
Maybe Vaughn LaRay Sandling
or
Paul LaRay Sandling
Another alternative is to name him a name that means four, you could take the number four in another language, or the name Verge has four in its meaning.
Verge LaRay Sandling
Fjerde (fourth in Norwegian-no idea how to pronounce)
Vierde (fourth in Dutch)
Viertel (fourth in German)
Cuarto (fourth in Spanish)
