You could combine a few of these mn choices to get Dorothy Janelle, Dorothy Janella, Dorothy Janetta or Dorothy Janette.
Kira is nice, but it just doesn't seem the best match for Dorothy. Ella and Etta are too open-ended with fn Dorothy, so they kind of sound like elaborations ... like Dorothiella and Dorothietta. Jana is ok, but I'd prefer one of the above alternatives, or simply Jane (Dorothy Jane is sweet). ;) Yakira is just nmsaa.

I voted for Dorothy Jana, which I like a lot. I just wanted to point out that in general, I think all the consonant names are going to flow better than the middle names that start with vowels. The similar ending/beginning sounds always tend to blur, and it's especially pronounced with these two-syllable vowel names, because they really sound like compound names (because -ella and -etta are frequent endings for female names -- Daniela, Gabriella, Marietta, etc)
When I say them out loud, it definitely sounds like Dorothiella and Dorothietta.
ETA: I didn't see Kelli's post before I wrote mine. I don't know if you are set on your middle name options, but if you're open to tweaking them, I love the idea of Dorothy Janelle or Dorothy Janette. And I LOVE Dorothy Jane.
