I'm thinking of using:

Lily O'Hara

I know Lily is tremendously popular but let's put that aside and just talk about the name itself. The other option I have is using Elisabeth O'Hara (Elizabeth is a family name) and calling her Lily but my mother is totally against putting an s in Elizabeth (but my husband & I prefer the s spelling) I'm just afriad she will always hate the way her grandchild's name is spelled. A big part of me thinks it shouldn't matter but then I remember how hard she is to get along with already. She is a most difficult mother.

I really like Lily even though it is popular. I do tend to prefer it as a nickname though, it just seems so cute that it should have something for formal to back it up. I bet your mother would get used to Elisabeth eventually, besides it SOUNDS like Elizabeth so it's not like she'd have to think about the pronounciation most of the time. If it really bothers her, she can always just use Lily.

Thanks. That's really nice of you to say. Actually, I'm branching out from family names with O'Hara. It's straight out of Margaret Mitchell's Gone With the Wind. It was Scarlett's last name (and I love, love, love Scarlett O'Hara!)-- and much softer than Scarlett. So I named my first little girl Charlotte (I thought Charlotte would weather better than Scarlett long- term) and hope to use O'Hara for the next one.

Does this help? The English form of spelling the name is Elizabeth. The French and German form is Elisabeth. This is one of the rare cases that it doesn't really bother me that the spellings are different (I hate spelling names different just for the sake of it), but so long as you are not sensitive to the fact that everybody else in the US will spell it with a Z, go for it. Let your MOM spell it any way she wants, but put it the way YOU want it on the birth certificate. If you nickname your daughter Lily, it probably won't matter anyway!

Yeah, I have one of those difficult mothers also.
