Family middle names? I think it's nice but not a must. Annelise's middle name was my grandmother's. I don't know if we'll be able to come up with another family related girl name that we like.
Middle names as first names? Both DH and I use our middle names as first names. He shares his first name with his father so he uses his middle name. He went through a most of public school using his first name but after HS went back to his mn. (His name is Kevin Brice.) I'm Catherine Vista for a couple of reasons. It sounds better than Vista Catherine.

We have chosen to use family names for our DDs' middle names, but I don't think it's necessary for every family. If we didn't have any names we liked for our first DD, we wouldn't have done it. My DH and I don't have family names for our middle names (although his first name is a family name).
My DH uses his mn as his fn b/c his name had a better flow that way, but his mom liked the mn better as a given name. We do not have any intentions of doing that for our children, but I don't think it's a terrible thing.
Our children will all have at least one middle name.
