Nathaniel took the STAR reading test (its how his school gauges what reading level they are on for the AR program) at the end of last school year.

And if he does? Well, I would put this in his IEP. Both for testing in the school as an accommodation, and for future state or college entry testing. I think it is great that his teacher realized that it is what he needed. Formalize it.
But, chances are, as his brain adapts to reading more and more, he will need the aloud time less and less. But if he doesn't that is fine. A teacher friend of mine was telling me how in college she was hired by the school to read the text book in an engineering class to a student. He could read a little, but not long paragraphs, but did fine while listening. He apparently had Pi memorized out to like 700 places. Yes this is different, but not a hindrance. The biggest problem may be other kids who don't understand and make fun of him.
Star Reading scores actually have a range, so the first two scores are not signiifcantly different. The best thing for you to do for your child is to have him read out loud everything at home, then, over time, lower his volume. By the end of the year he should be reading with only his lips moving. He will benefit from the reading aloud, and wean down the scaffold during the year.
