ADHD or Just hyper and mean

My son will be 5 in a few weeks.  Last year he was in preschool and he got in trouble a couple times for getting mad because he didn't get his way.  Once he punched a mirror at school and hit the wall a couple times.  At home he was whining and screaming all the time when he didn't get his way.  Over the summer he has got worse.  Timeout didn't work.  He would put himself in the corner while he was yelling at us.  If he doesn't get his way then he throws the biggest fits.  He used to throw himself on the floor and cry.  Now he yells back at us and tells us he hates us and doesn't like us anymore.  He threatens to run away.  He has actually tried to choak me because I was outside talking to a neighbor and he wanted me to come in.  He wanted me to get him chocolate milk one night and I didn't get up right away so he went and got a small kids knife and threatened me with it.  He is very angry.  But then in a matter of minutes he is the most loving and cuddling little boy who wants to kiss me to death and won't leave my side.  My husband has parkinsons and can not handle him running back and forth in the house like he does.  He is usually playing with invisible bad guys.  Runs back and forth through our small trailer.  His dad can not handle it.  I have tried disciplining him in several different ways.  I just don't know what to do anymore.  He doesn't listen at all.  He is in preschool again this year because his birthday is so late.  They are not having any trouble with him yet.  He is super smart and can finish a project wtih not problem.  He can watch a movie.  He is already starting to read and spell.  I always thought ADHD symptoms where trouble learning because they couldn't concentrate.  He remembers everything.  If anyone has any recommendations I really need the help.  I don't know wether to take him to a counselor or back to the doctor...  Last time we tried the counselor she told us we needed teh counseling because we weren't parenting right.  Yet everything she told me we needed to do we were already doing.  Thank you!

That's a good suggestion about keeping a diary, that could shed a lot of light on what's behind his behavior.
