My ds struggled swallowing his Concerta at first, his are like what you described.
I'd try the practice thing, too--but if that fails, you could always get a script for 2x18mg Concerta. The things are additive, so that's the same as 36, and the pills are a LOT smaller.
Concerta never lasted 12 hrs for my kid either--entire school day, occasionally plus a little. For homework, we use "boosters"--small dose of plain ritalin (not time-release) to increase the med level in the body. Only on days with loads of homework, of course!
My son is on Concerta, as well. We were told it would likely start to wear off after about 8 - 10 hours.
Hope your son does well on the alternative forms of the meds.
I would start practicing with him, with M&M's or something similar. It will be alot easier if he can swallow pills.
Glad you are going to call the Dr, chewing the Concerta could be dangerous.
A child may HAVE ADHD, but it is not what they ARE. Never tell a child they ARE ADHD.

Is he refusing to swallow it, or just cannot? You can practice with M&M's and tic tacs. Thats how alot of kids learn to swallow pills.
Concerta should not be chewed, or opened. It won't release the right way. Basically, he is probably getting approx 30 Mgs all at once, which is a HUGE dose. It is probably only lasting 4-6 hours, and that is why he is having side effects and problems. He is having problems with homeowrk, because there are no meds in his system.
