My 5.5 year old has combined type (Am I right? He is impulsive and overactive).
One of his main problems (now) is being too loud at school.
My kid is a "loud-talker" as well and I've been trying to get him to control the volume for years. My SS (who is an adult), my DH, and all his brothers, and my sister are all "loud-talkers" (and all of them function very well in society). Some people just are, and I think that it's really, really hard for them to control it for very long.
The teacher is just going to have to be patient and consistent about reminding him to use his "inside voice". There is no magic cure, and there really isn't anything you can do about his volume at school unless you are also there in the classroom. You can work on him at home when he's too loud in the house, but in this case, the loudness at school is kind of the teacher's problem, if you kwim.
Did the doctor check his ears? Since this started more recently perhaps he has a build up of wax or other stuff (doctor found popcorn in my daughter's ear when she was 4. How did that get there???) It couldn't hurt to have his hearing checked to make sure that he is hearing properly. If that doesn't help, perhaps talk to the school speech therapist to see if there is a way that he/she can help, including helping the teacher.
I think ADHD combined is Hyperactive and Inattentive.

Speaking as a doctor who treats ADD/ADHD, and as a mother of a son with ADHD, I hear your concerns about volume (no pun intended!). My son was the same way. As he became more excited or agitated, he would get very loud for such a little guy. And yes, it is a sign and
Yes, you are right about combined type.
Loud--I think the earwax check is probably worth doing, if it's recent. Also, my ADHDer used to be loud. We had to explicitly teach the "quieter" thing. He didn't "get" it from people saying "inside voice", "quieter", etc., until we had him repeating what he said over & over to get it quieter. Then he "got" it & could do it. No clue if that's a potential problem for you--you can certainly have a kid who is simply impulsively forgetting!
Concerta 18--remember it's also possible that Concerta 18 isn't "enough" either, if you want to bump up there instead of swapping back to the Adderall. But like you said--one day doesn't make for enough info!
