We started this in 2nd grade and continued last year w/ lots of supports from the OT, this year he hasn't had as much difficulty focusing and using inappropriate stims.

Christopher's OT at school used this program with him in fifth grade, I think (he's in seventh now). She actually photocopied the booklet for us so we knew what was happening. I don't know how "successful" it was, but we do sometimes use the "engine running" terminology to help him focus on how he's feeling.
