Grrrrr....
First, in my letter requesting the FBA, I requested that he be observed during group work periods, at recess and lining up....any time he may get into trouble with a classmate. He was only observed in the classroom and on St Patrick's day which isn't a typical day at all.
The student characteristics section was so vague and one dimensional, surface description of Sam.
It was check this kind of report/form
Under Setting variables/long term triggers he checked
prior social conflict - peers
transitions
noise
excessive activity
He did not check
fatigue
fear of failure
change in staffing
anxiety
or change in routine which I think greatly impact Sam. But he did only see him on one day and did not get any parent input.
For function of behaviors he checked
task/classroom avoidance
affective regulateion
attention- adults and peers
social skills deficitc
He did not check communication deficits or academic frustration which I know are sometimes a problem.
Replacement Behaviors - Sam would benefit from learning how to work independently, how to pay attention and complete assignments and learning social/communication skills... Duh!
Positive behavioral supports - social skills training, self monitoring to assist in academic activities, sensory eval......a smaller class may help him be less distracted.

Theraputic supports he only checked social skills training.....but nothing else on the list applies to Sam (mostly drug intervention and psych. therapy stuff.)
I'm most concerned about the lack of observation at recess and the restriction of movement statement......that seems to go against his recommendation of sensory concerns. Plus, I know Sam restricting his movement won't help. Unless he means restricting transitions, but wouldn't the report say transitions??!
UGh....grrr.....argh......I'm reduced to grunting I'm so frustrated!!!!

Well, check marks and a half done FBA, I would likely request an independent assessment. It will take longer but it is better than having an incorrect one trust me.
I had requested an IEE for our FBA last year. However, Mike's teacher is great and he started doing better. They were having a hard time finding someone to do the FBA and because he started to do better in the process, I dropped the ball so it wasn't done. Now I am having to start over from scratch to get his current FBA changed or taken out of his IEP. Pain in the tush. He is still doing ok I just don't want the inaccurate stuff in his file. But it has been a major pain in the tush trying to get it done. I should have pushed the IEE last year instead.

Your son's FBA reminds me of my friend's son's first FBA.......it was done by the Spec Ed director of our district......it was completely worthless. Several years later she had another done by a specialist from the autism center at the Cleveland Clinic. This specialist actually uses my friend's son's first FBA (with all personal info blacked out) as her example of "what NOT to do" on a FBA for her staff. We are in the process of having this same specialist do my son's FBA.....she just started her observations this past week. The FBA she did for my friend had over 60 hours of observation (not just in the classroom but at home and at activities and therapy appts as well) and the final report was over 40 pages long.....very indepth and detailed.
I personally know that I am not trusting our school district to do a proper FBA so I am having this outside source do it.......just my $0.02
