I have a feeling of deja-vu when I ask this question but have you tried sensory interventions? When Peter was younger he was very unmanageable at home. He would come off the bus and either start bouncing off the walls or else he woudl hide is a tiny dark quiet space. Both were sensory needs.
I started an agressive sensory OT program at home which included a lot of heavy work, squishing with a therapy ball. Making him a safe "housie" out of an old TV box filled with stuffed toys and cushions, and as much outdoor/running around time as possible.
It helped us a lot
It could be something to try before you go the medication route. There are several books available regarding sensory needs

Hi Paula!
Mike picks him up every day after school and on most days (unless it is raining), they go to the playground and climb, run, play kickball for about 45 minutes until Nick gets tired.

My Nic is on 20mg's of Prozac and he doesn't have any side effects from it.
