I'm going to try to give a fast overview of my son. He is a former 26 week preemie.

Your son sounds a lot like my son in terms of listening and understanding. It is due to a receptive language delay. Is he getting speech therapy or are you trying to get it for him? That could help a lot. There are also

Megan used to have a lot of the same issues - it's gotten way better over time. She still often will ignore us when we talk to her - but then if we say something she cares about, she'll immediately react. But sometimes it does seem like she doesn't understand what we're saying to her. A child can have a processing disorder - where things just seem jumbled to them when they are hearing it.

Megan's gone through different things - like sucking and chewing on her fingers, tearing up paper into little bits. I think it's a calming thing to her. She also sucks and chews on blankets. She used to do it on towels and other cloth. Our OT has helped us with ideas on redirecting that energy into something better. If your son is seeking sensory input, sometimes kids like to put their hands in a bowl of dry beans. You can hide toys in there. Anyway, I would ask an OT.

Sometimes when kids have delays, they have really scattered skills - in some areas they can be typical, and others - things just are very delayed. It doesn't mean those areas won't come along in time. Sometimes the brain I think just needs to develop, and OT and other therapies can help.

You might also check out Stanley Greenspan's book - the Child with Special needs - and the chapters on floor time. I just borrowed it from the library.
