Sorry, I don't have time to cover everything, but as far as the surgery goes, can you take her to see a developmental optometrist before the surgery and see if vision therapy can do the same thing?

With regards to the IEP, you will need the diagnosis before you can proceed down that path. A 504 isn't as stringent, it's basically just a plan the teachers have to follow with regards to your child, on of my boys has one. You should discuss it with the school, it never hurts to have some protections in place. Hopefully your school will be receptive to the putting some type of plan in action for her.
You don't need a diagnosis for an IEP, although it helps. The parent just needs to write a letter to the school saying there appear to be issues interfering with learning. The school will then do their own evaluation to see if the child qualifies for services or accommodations, but they will not give you a diagnosis.
