Special Needs and the Dentist? Sigh.

Hi everyone. I hope you don't mind me stopping in. I have an 8yo boy who is ADHD and learning/communication disabled. We've had a long road thus far; an emergency brain surgery when he was 1, behavior issues, learning and socialization issues, and fears.
Which brings me to my current situation. At the beginning of the year he had to go to the dentist. He had a cavity. He had been to the dentist once before, but ever since, he wouldn't sit and it became so frustrating that we'd just leave. So we went to the ped dentist and got things taken care of. Of course, it didn't go without incident. He's a big boy, 4'3" and 90 pounds (I'm only 5'5 so, he's not far behind me and he's only 3rd grade). We paid for the knock-out medicine which didn't knock him out (yeah, waste of $400). He had to be papoosed, which didn't go so well (lots of screaming and kicking and spitting). But we got things done. However, last night I noticed a hole/chip on his first molar. I don't remember if it was a tooth that was filled and it fell out, or not. So now I'm not sure what to do. If he didn't have this fear and didn't act out in self-defense, I would take him in a heartbeat. But now I'm stuck weighing the pros and cons.
Does anyone have any experience with something like this? Any ideas on how to get through it? Any thoughts at all?
